Nursing Home Transition

The Nursing Home Transition program is a priority at DON and is provided to individuals residing in nursing facilities who choose to live independently. This service assists people with disabilities in finding accessible housing, transportation, personal assistance services, and other community resources necessary for full inclusion into society.

The Nursing Home Transition program is a comprehensive coordination of services and resources that provide alternative living arrangements to those residing in a nursing facility or other institutional settings.

Steps include:

1. DON determines if you meet the qualifications for a safe and healthy transition

2. DON acts as a liaison between the facility and you, the consumer

3. DON assists in developing an integrated plan that meets the individual's needs

4. DON assists in finding and securing housing (including needed household goods)

5. DON provides community resource information

6. DON coordinates the date for discharge
